Breast Augmentation: With Lift or Without?

Sometimes, particularly after weight loss or breastfeeding but also just because of the effects of gravity, the breast tissue and nipple areola complex come to sit in a “droopy” position. Whilst in some such patients it may be necessary to both use a breast implant and perform a lift, other patients can get away with having a pure breast augmentation (i.e. insertion of implants only). For patients having a breast augmentation alone, Dr. Richardson recommends an anatomic or teardrop-shaped implant placed on top of the muscle. Placing it on top of the muscle prevents the longer-term complication of the breast dropping off the front of the implant and leaving the patient with one mound, which is the implant, along with another formed by natural breast tissue that sits at a lower level. If the implant is put on top of the muscle, it tends to move with the breast as it naturally descends under the influence of gravity. The teardrop-shaped implant has a lower centre of mass and therefore causes the nipple to project more outward and upward; by contrast, a round implant, which often has a higher centre of pointed projection, and can under some circumstances be pushed in a downward direction.

The advantage of using an implant alone to create volume is that doing so avoids the scars involved with a breast lift. In some circumstances, though, it is necessary to perform a breast lift along with the breast augmentation to bring the breast and nipple complex to a higher level so that it is more centred over the furthest-projecting part of the breast implants. Dr. Richardson normally uses a round implant for the combined breast augmentation and lift procedure at his Queensland practice, although a teardrop-shaped implant can be used, and the implant can be placed on top of the muscle or behind the muscle according to the breasts’ characteristics and the patient’s wishes. One of the downsides of having a breast lift is the scars associated with it; at a minimum, these run all the way around the nipple-areola complex, and they may also run vertically down to the inframammary fold and horizontally inside of it.
