What is involved with breast implant removal surgery?

Breast implant removal surgery involves reopening the breast implant pocket and removing existing breast implants. There are two different approaches to breast implant removal and these are defined by their interference with the breast implant capsule during the procedure.

Standard Removal

A standard breast implant removal procedure (also referred to as partial removal) involves only removing the breast implants, while leaving the capsule (scar tissue surrounding the implant) intact within the breast pocket. This is the most common approach for breast implant removal.

Full Capsulectomy (Total Removal)

This approach involves taking out both the breast implants as well as the surrounding scar tissue (capsule).

Following surgery, patients will be provided with specific post-operative aftercare instructions to support optimum healing and recovery. Breast Implant Removal Incisions & Scarring

The type of incisions and resulting scars from breast implant removal surgery depend on factors such as the surgical technique used, whether a capsulectomy (removal of scar tissue around the implant) is performed, and if additional procedures like a mastopexy (breast lift) are included.

Common Incision Types & Scarring:

Inframammary Incision (Under the Breast Fold)

  • Location: Along the natural crease under the breast (inframammary fold)
  • Scarring: Typically well-hidden in the fold, making it less visible
  • Used for: Standard implant removal, breast implants are often removed via the same incision as the original breast augmentation

Peri-areolar Incision (Around the Nipple)

  • Location: Around the outer edge of the areola.
  • Scarring: Blends with the natural colour transition of the areola, making it less noticeable
  • Used for: When combined with a mastopexy (breast lift) or in cases with minimal scar tissue

En Bloc or Total Capsulectomy Incision

  • Location: Often a longer inframammary or vertical incision to fully remove the implant and surrounding scar tissue
  • Scarring: More visible than a standard removal but necessary for complete removal of the capsule
  • Used for: Cases of implant rupture, severe capsular contracture, or BII (Breast Implant Illness) concerns

Anchor or Lollipop Incision (With Breast Lift)

Your Breast Implant Removal Consultation

The consultation is a crucial first step when considering any surgical procedure. Before proceeding with any elective cosmetic procedure, all patients are required to obtain a referral from their GP or specialist, complete a pre-consultation body dysmorphic disorder screening questionnaire, and attend at least two pre-operative consultations.

What is the pre-consultation body dysmorphic disorder screening questionnaire?

It is an AHPRA requirement that all patients considering cosmetic surgery must complete a Body Dysmorphic Disorder questionnaire. This is to determine suitability for surgery and to ensure patients have realistic expectations regarding the outcome of their procedure prior to undergoing surgery.

Initial Consultation (mandatory)

On arrival for your initial consultation at Brisbane Plastic & Cosmetic Surgery, you will be welcomed in-clinic by our patient care team who will assist you with any required pre-consultation paperwork, reports and referrals prior to seeing your surgeon.

During your initial consultation, you will meet with your surgeon to discuss your goals, concerns and motivations for surgery as well as associated risks and realistic outcomes. This consultation gives you an opportunity to learn more about your chosen surgeon and their experience, and get all of your questions answered about what surgical approach is most appropriate for you. Your surgeon will review your medical history and discuss your aesthetic goals, and provide you with comprehensive information about your breast implant removal procedure, including recovery times, and the potential risks and complications associated, and how your surgeon plans to manage these, should any complications arise. Subsequent Consultations (as required)

Additional consultations for your procedure are at no extra cost and can be conducted face-to-face or online via video call.

Pre-Operative Nurse Appointment (mandatory)

Once a surgery date has been booked, our patient care team will organise your pre-operative appointment, which will be with one of our nurses, and scheduled 2 weeks prior to your booked surgery date. The purpose of this appointment is to run through your surgery plan and take your pre-operative images. It will be another opportunity for you to ask any additional questions you may have leading up to your surgery, as well as complete any necessary paperwork to ensure you are fully prepared for your upcoming procedure with us.

Breast Implant Removal Surgery

During breast implant removal surgery, your surgeon will make an incision in the inframammary fold to access the implant and capsule. If your implants were originally placed through this incision, the existing scar can often be reused without needing to extend it. If the implants were inserted via an areola incision, it may also be possible to use the same incision for access. Depending on your specific case, a partial or total capsulectomy will be performed. The most appropriate surgical approach will be discussed with you during your consultation.

Breast Implant Removal surgery is performed under general anaesthetic, and in a fully accredited private hospital in Brisbane. General anaesthesia is administered by an anaesthesiologist – a registered medical practitioner fully trained to administer anaesthetic and monitor the patient while they are induced. In some cases, patients may be advised to stay overnight or may opt to. Patient’s will be advised during their consultation if an overnight stay is recommended.

Following breast implant removal, patients will be given a support bra or elasticated bandage to minimise swelling and provide additional support during the healing process. In some cases, a small, temporary tube may be inserted to drain excess fluid or blood. Patients will be provided with specific post-operative instructions to aid in a smooth recovery.

As with any surgical procedure, breast implant removal surgery carries potential risks and complications. These may vary depending on individual factors, including overall health, the presence of scar tissue, and the surgical approach used. Breast Implant Removal Recovery

Recovery from breast implant removal surgery will vary from patient to patient, and depends on the extent and complexity of the procedure and the patient’s individual healing capacity.

Patients may experience pain, aches, pressure and discomfort, generally lasting up to two weeks after breast implant removal. Pain relief medication can help address discomfort after breast explant surgery. Pain management will be at the discretion of your surgeon and your anaesthetist, and we will be able to prescribe oral pain medication to assist with any pain and discomfort.

During recovery, patients will be advised to:

  • Keep the incisions clean and dry
  • Avoid smoking
  • Drink plenty of water
  • Eat nutritious food
  • Refrain from strenuous tasks
  • Take time to rest and recover
  • Avoid underwire bras
  • Avoid lying on your side for a few days
  • Wear compression garments for at least two weeks

Frequently Asked Questions

How will I know if my implants are ruptured?

If a silicone implant ruptures, the new cohesive gel should remain within the outer shell and not harm your body. It is hard to detect a rupture so an ultrasound may be necessary if suspected.

Do implants need to be replaced regularly?

Breast implants have a limited lifespan and at some stage you may require additional surgeries to replace, upsize or downsize, reposition, or you may decide to get the implants removed entirely. Generally, it is recommended that breast implants are replaced every 10-15 years.

The time that it takes for a breast implant to reach the end of its lifespan varies between patients and some may even last up to 20 years without needing to replace them. It’s all a case of monitoring the shape and appearance of your breasts and scheduling regular check-ups.

What happens if you don’t replace your implants?

Implant rupture or capsular contracture can occur in some patients who leave their implants for too long. That said, some patients have been known to have their implants for over 20 years with no issues or complications. It is recommended that patients with breast implants get their implants checked yearly to every 2 to 3 years.

Will removing my breast implants lift my breasts?

Breast implant removal surgery will not help lift sagging breasts. Patients who undergo breast implant removal surgery may combine the surgery with a mastopexy (breast lift) to lift the breast and remove any excess tissue.

Yes, the Medicare rebate for Breast Implant Removal Surgery is under Medicare Item Number 45551 – removal only.

What is the breast implant capsule?

When a breast augmentation mammoplasty is performed, the patient’s body will respond to the breast implants by forming a “capsule” around the implant. This breast implant capsule is made from scar tissue and collagen fibres. The capsule plays an important role in stabilising the breast implant and maintaining its position. In some cases, if too much scar tissue develops capsular contracture can arise.

What is explant surgery?

Explant surgery involves an incision made along the inframammary fold (usually guided by the incision from the original breast surgery), and through the implant, capsule to remove the breast implant. This approach leaves the capsule in the pocket where it naturally shrinks in size and reabsorbs naturally.

What is a full capsulectomy or an en bloc capsulectomy?

A full capsulectomy involves the removal of the breast implant and subsequent removal of the capsule. An en bloc capsulectomy refers to the removal of the breast implant and capsule together at the same time. The en bloc capsulectomy is Dr. Richardson’s preferred method of capsulectomy. Some patients may not be a suitable candidate for en bloc capsulectomy depending on the state of their implants and the capsule, as well as the proximity of the capsule to the rib cage. Our specialist plastic surgeons will always choose to take the safest route for each patient and their circumstances.

Why would a capsulectomy be recommended instead of standard breast implant removal?

A capsulectomy will be recommended in a number of circumstances. Firstly, patients can request a full capsulectomy and they may do this due to personal preference or peace of mind. More commonly, capsulectomy is recommended to patients that have intracapsular rupture and a capsulectomy is the most effective way to remove any leakages. If a patient has capsular contracture where the capsule is restricted and excessive scar tissue has formed, a full capsulectomy may be most appropriate. In extremely rare cases of conditions such as BIA-ALCL, the removal of the capsule is also recommended.

What happens if I leave the capsule?

The most common approach to explant surgery is the standard approach, leaving the capsule. If the capsule is normal, thin, and there are no complications such as capsular contracture or rupture then the capsule will shrink and may leave some minimal residual scar tissue. This should not affect the aesthetics of the breast nor interfere with mammograms in the future.
